Insurance Assistance
We accept most dental insurance plans. (Click on Participating Insurance Plans to see some of the insurance plans in which we participate.) Moreover, we accept an assignment of benefits from most insurance carriers. For your convenience, we will submit your claims directly to your insurance company. If you have questions concerning your dental insurance, please contact the business department of the Midwestern Dental ® office you attend.

Financial Obligation
To the extent possible, once we have completed your initial exam, we will review our recommended treatment plan and its total cost* with you before performing any major work.

Payment Options
We accept payment in cash and by check, credit cards (Visa, MasterCard, Discover and American Express) and debit cards.

Long-Term Payment Plan
For larger treatment plans, we offer a financing option to help you pay for your dental treatment called CHASE HEALTH ADVANCE. Features of this option are:


No down payment

No application fees

No prepayment penalty

Us our "up to 18 months interest free" option

Select monthly payment that fits within your budget

Keep your credit cards available for “other” purposes

dental treatment of $300 to $12,000.  Offering CHASE HEALTH ADVANCEallows
us to make the smile you've always wanted affordable.
* Your cost is based upon the amount we expect your insurance
company to pay. Your cost will be higher to the extent it pays less.
